I understand that the deposit will be applied toward my pet's scheduled services, but will NOT be refunded if I do not show up or cancel less than 72 hours before the appointment.  My appointment is not confirmed until I sign the Consent Form.

Prior to my pet's scheduled appointment, I will:

• read the Pre and Post Op Instructions listed on this website

• will look at the Check In / Check Out section

• will email SPCA of Lake County with any questions
